Nursing Home Transition and
Diversion Waiver Services

The Nursing Home Transition and Diversion Medicaid waiver offers options available to New York state residents who are aged eighteen to sixty five. These services are for people who are currently disabled or of the senior age who would like to continue to receive services in the community rather than in a nursing home. There is no specific criteria needed to be eligible for this service. FLIC works in collaboration with the local Regional Resource Development Center for the Nursing Home Transition and Diversion Waiver service.
